醫(yī)學(xué)免費(fèi)論文:血清Galectin-3對(duì)胰腺癌診斷價(jià)值的初探
【摘要】 目的：探討聯(lián)合檢測(cè)血清半乳糖血凝素-3(galectin-3)水平、癌胚抗原(Carcinoembryonic Antigen，CEA)和糖鏈抗原199(Carbohydrate Antigen 199，CA199)對(duì)胰腺癌的互補(bǔ)診斷價(jià)值。方法：ELISA法檢測(cè)胰腺癌30例、胃腸道腫瘤18例、胰腺炎13例、正常人16例血清galectin-3濃度，同步測(cè)定血清CA199及CEA，比較galectin-3與CA199、CEA診斷胰腺癌的靈敏性及特異性， galectin-3與CA199和CEA對(duì)胰腺癌的互補(bǔ)診斷價(jià)值。結(jié)果：(1)血清galectin-3在胰腺癌患者的檢出率為30.0%，而胃腸道腫瘤、胰腺炎、正常人檢出率為0%;(2)ROC曲線分別確定galectin-3、CEA及CA199對(duì)良、惡性胰腺疾病的鑒別界值分別為0 ng/ml 、5.2 ng/ml 及35 U/ml。Galectin-3、CEA及CA199在胰腺癌中診斷陽(yáng)性率分別為33.3%、50.0%及63.3%;(3)血清galectin-3與CEA及CA199間均無相關(guān)性 (P均>0.05)，聯(lián)合檢測(cè)galectin-3、CEA及CA199對(duì)胰腺癌診斷具有互補(bǔ)價(jià)值。結(jié)論： 胰腺癌患者血清Galectin-3升高, 聯(lián)合檢測(cè)Galectin-3與CA199和CEA可提高對(duì)胰腺癌的診斷敏感性。

The significance of serum galectin-3 in the diagnosis of pancreatic cancer

[Abstract] Objective: To study the serum galectin-3 level in the cases with pancreatic cancer and to explore the complementary value of galectin-3, CEA and CA199 in the diagnosis of pancreatic cancer. Methods: Serum galectin-3 were measured with ELISA in 30 cases with pancreatic cancer, 18 cases with gastrointestinal cancers, 13 cases with pancreatitis and 16 healthy persons. CA199 and CEA were measured with chemiluminescence immunoassay. The sensitivity and specificity of galectin-3, CA199 and CEA for diagnosis of pancreatic cancer were compared. The complementary value of galectin-3, CA199 and CEA for diagnosis of pancreatic cancer was also studied. Results：(1)The detection rate of galectin-3 was 30.0% in cases of pancreatic cancer, while 0% in cases with gastrointestinal cancers, pancreastitis and healthy persons. (2)Based on ROC curve, the cut-off value of galectin-3, CEA and CA199 for differentiating benign and malignant pancreatic diseases were 0 ng/ml, 5.2 ng/ml and 35 U/ml respectively. The diagnostic positive rates of galectin-3, CEA and CA199 for pancreatic cancer were 33.3%, 50.0% and 63.3% respectively. (3)Galectin-3 does not correlated with CEA or CA199(P>0.05). Combined determination of Galectin-3, CEA and CA199 are complementary to the diagnosis of pancreatic cancer. Conclusions: Serum galectin-3 increases in the cases with pancreatic cancer. Combined determination of serum galectin-3, CA199 and CEA may increase the diagnostic sensitivity for pancreatic cancer.

半乳糖血凝素-3(galectin-3)是半乳凝素家族的一員，是能識(shí)別糖蛋白和糖脂的特異性低聚糖結(jié)構(gòu)。galectin-3在某些腫瘤組織中高表達(dá)[1-3]，其在腫瘤的發(fā)生與發(fā)展過程中所起的作用正日益受到重視。我們?cè)ㄟ^雙向電泳及MALDI-TOP-TOP質(zhì)譜分析技術(shù)，對(duì)胰腺癌組織和正常胰腺組織蛋白質(zhì)組進(jìn)行對(duì)比分析，最終質(zhì)譜鑒定得到胰腺癌的16個(gè)差異表達(dá)的蛋白質(zhì)，其中g(shù)alectin-3即為表達(dá)上調(diào)蛋白之一[4]。迄今為止，尚未見胰腺癌患者血清galectin-3的研究報(bào)道。本研究檢測(cè)了良惡性胰腺疾病及其它惡性腫瘤患者血清galectin-3水平，研究血清galectin-3檢測(cè)對(duì)胰腺癌的診斷價(jià)值及galectin-3與CEA與CA199診斷胰腺癌的互補(bǔ)診斷價(jià)值。
