術(shù)前月經(jīng)失調(diào)（閉經(jīng)、月經(jīng)量少、不規(guī)則出血等)，術(shù)后8-9個(gè)月后可以恢復(fù)正常。肝移植術(shù)后懷孕生育也時(shí)有報(bào)道[49]。有人認(rèn)為，肝移植術(shù)后女性完全可以恢復(fù)正常的生育能力，但早產(chǎn)和剖宮產(chǎn)的概率較高。在懷孕期間，免疫抑制應(yīng)該正常使用，但要注意監(jiān)測(cè)。至目前為止，還沒(méi)有致畸的報(bào)道。懷孕及生育不會(huì)增加排異反應(yīng)的危險(xiǎn)[50]。由于口服避孕藥會(huì)增加血液粘稠度，加重高血壓及藥物之間的反應(yīng)，故肝移植術(shù)后病人不宜使用口服避孕藥。
肝移植術(shù)后對(duì)男性生育能力的影響還未見(jiàn)報(bào)道。由于肝移植病人的下一代均未到生育年齡，因此對(duì)下一代生育能力有無(wú)影響也未見(jiàn)報(bào)道。
