§1 Drop-method Instilling Ophthalmic Drops Today we are going to learn a new medication administration, that is drop-methed. The drop-methed includes 3 kinds of it.: first is instilling ophthalmic drops. second is administering otic medications. Third is instilling nose drops. Now lets begin to learn the method of instilling ophthalmic drops. Objectives First is objectives. (I think the objectives of instilling ophthalmic drops is simple). There are tow objectives: One. is to prevent and treat the ophthalmocace(eye disease); the other is to be used before and after eye operation. Preparation We know before any procedure of medication administration nurses have to prepare everything well carefully. How to prepare for it before procedure. First is the preparation of equipment, equipments are; Eye medication (in ocumeter container) and Cotton ball or tissue Second preparation is for procedure, there are several steps 1. Obtain patients medication record.(medication record may be a drug card, medication sheet) Remember to compare the medication record with physicians order. Rational is safety check 2. Wash your hands. 3. Take the medication from the drug box on medication trolley. And also remember to compare the label on the medication bottle to the medication record for safety check. 4. Gather necessary equipment. Procedure After the preparations the nurse takes medication to patients room. Now the procedure : 1. 1. To check patients bed number against medication record、and also check patients identaband, then ask patient to state name. All these you do just for safety check. Do you know identaband?identaband is a band wear around the wrist ,on the band there are bed number and patient’s name. 2. 2. To explain procedure to patient. You know that is necessary. 3. 3. To wash醫(yī).學(xué)全在線gydjdsj.org.cn your hands. That is for prevention of cross-contamination. 4. 4. To tilt patients head slightly backward and ask patient to look up. Rational is the cornea is protected as patient looks up. 5. 5. Uncap ocumeter, placing cap on its side 15 6. Give tissue to patient for wiping off excess medication. 15 7. Place ocumeter container1/2~3/4 inch(1~2cm) above eyeball with dominant hand Rational: this position reduces risk of ocumeter container touching eyeball and causing injury. 8.Stabilize hand holding ocumeter container as necessary. Place nondominant hand on cheekbone and hand holding ocumeter container on top 9. Expose lower conjunctival sac (sauk) by pulling down on cheek, creating a “cup” 10.Drop prescribed number of drops into center of conjunctival sac. Remember: not to place medication directly on cornea. That could cause injury to cornea. 11 .Ask patient to gently close eyelids and move eyes. Rationale is it can distribute solution over conjunctival surface and anterior eyeball. 12.presse lacrimal sac for 1~2min.rationale is to avoid eye medication flow to nasolacrimal duct and nasol cavity. Very gently massage closed lid for patient who cannot cooperate(e.g,comatose). 13. Remove excess medication with tissue 14.
