【摘要】 目的 探討美托洛爾對(duì)充血性心力衰竭(心衰)大鼠血漿腫瘤壞死因子-α(TNF-α)的影響。方法 雄性SD大鼠80只,隨機(jī)分為空白組(n=10)及模型組(n=70),模型組皮下注射異丙基腎上腺素2次(170 mg· kg-1),6周后應(yīng)用超聲心動(dòng)圖檢測,以左室射血分?jǐn)?shù)≤45%確定造模成功,將心衰大鼠模型隨機(jī)分為心衰對(duì)照組(n=13)、美托洛爾組(n=14);18周后使用左心導(dǎo)管進(jìn)行血流動(dòng)力學(xué)檢查,利用放免法檢測血漿TNF-α的改變并觀察心肌病理形態(tài)學(xué)特征。結(jié)果 ①與空白組比較,心衰對(duì)照組心功能明顯惡化,心室質(zhì)量指數(shù)、左心室質(zhì)量指數(shù)及血漿TNF-α顯著升高(P<0.01 ),心室重構(gòu)明顯;②與心衰對(duì)照組比較,美托洛爾組心功能明顯好轉(zhuǎn),心室質(zhì)量指數(shù)、左心室質(zhì)量指數(shù)及血漿TNF-α明顯降低(P<0.05 ),心室重構(gòu)好轉(zhuǎn)。結(jié)論 美托洛爾能改善心功能,逆轉(zhuǎn)心室重構(gòu),其作用可能與抑制心衰大鼠血漿TNF-α表達(dá)有關(guān)。

Effect of metoprolol on plasma TNF-α in rat model with isoproterenol-inducedcongestive heart failure

Abstract: Objective To explore the effect of metoprolol on plasma tumour necrosis factor-α (TNF-α) in rats with congestive heart failure (CHF). Methods 80 male SD rats were randomly allocated to the isoproterenol (ISO)-induced heart failure group (n=70) receiving a subcutaneous injection of ISO (170 mg per kilogram of body weight) twice and the healthy normal group (n=10) receiving a subcutaneous injection of 0.25 ml of normal saline twice. At the 6th week, the left ventricular ejection fractions (LVEF) were measured by echocardiogram, the remaining rats, with a LVEF≤45%, were randomly divided into two groups: heart failure control group (n=13), metoprolol group (n=14,8 mg/kg-1· d-1, i. g., ). At the 18th week, the hemodynamic parameters were detected, including HR, LVEDP, LVSP and ±dp/dtmax; LVWI was measured after LV morphopathological examination. TNF-α in plasma was examined by radioimmunoassay. Results ①Compared with the normal group, the heart function significantly deteriorated; VWI, LVWI and the plasma TNF-α significantly increased (P<0.01) in the heart failure control group. ②Compared with the heart failure control group, the heart function markedly improved, and the VWI, LVWI and the plasma TNF-α decreased significantly (P<0.05 ) in the metoprolol group, with an improved ventricular remodeling. Conclusion Metoprolol can inhibit the plasma TNF-α, improve heart function and reverse ventricular remodeling with CHF. 充血性心力衰竭(congestive heart failure, CHF,心衰)是各種心血管疾病最終的共同轉(zhuǎn)歸,嚴(yán)重威脅著人類的健康。目前研究認(rèn)為心衰是由于神經(jīng)內(nèi)分泌細(xì)胞因子系統(tǒng)的激活而介導(dǎo)的心室重塑過程[1]。在心衰的發(fā)生發(fā)展過程中,血漿腫瘤壞死因子-α(TNF-α)具有降低心功能、促進(jìn)心衰發(fā)展的作用[2-3]。本研究以心衰大鼠為實(shí)驗(yàn)?zāi)P,觀察美托洛爾對(duì)血漿TNF-α 的影響并探討其意義。
